Eligibility: 1st of the month following 30 days of employment, must be a Full-Time employee with FTE of .75/30 hours per week or more.
Benefits: Exams, lenses or contacts, and frame benefits are based on a calendar year not date of last service.

This plan also includes retinal imaging and a diabetic eye care benefit.
Retinal imaging – is an alternative to dilation during an exam. It provides a high-resolution image inside the eye and can help with early detection of serious eye conditions such as glaucoma, high blood pressure, and cancer. Imaging will be available to members at the same frequency as exams. This covered benefit will have a member responsibility of no more than $39. According to EyeMed, members are more likely to choose retinal imaging over dilation.
Diabetic benefits – additional eye care benefits will also be available for members with Type 1 and Type 2 diabetes. The enhancement allows for additional exams and diagnostic testing to monitor signs of any diabetic ocular changes at no out-of-pocket cost to the member beyond their basic exam. Diabetes is the leading cause of blindness in adults; early detection, timely treatment, and appropriate follow-up care of diabetic eye disease can protect against vision loss.
