Who is Benefit Eligible?
Eligibility for the benefits provided by IncredibleBank depends on your employment status. Employees working 30 or more hours per week and classified as a .75 FTE or higher are eligible for benefits.
Eligible dependents are generally regarded as:
- Your legal spouse
- Your children under age 26, including stepchildren, legally adopted children and children placed with you for adoption
- Your dependent children of any age who are physically or mentally unable to care for themselves
When Benefit Changes Can Be Made
Due to IRS regulations, once you have made your election for the 2025 plan year, you cannot change your benefits until the next annual enrollment period unless you experience a qualifying life event.
A qualifying life event may include the following:
- Marriage, legal separation, divorce, birth or adoption of a child;
- Change in employment status for you or your spouse;
- Change in a dependent’s benefits eligibility status;
- A significant change in the cost or coverage of your spouse’s benefits;
- Change in place of residence causing a loss of eligibility;
- Change in the cost of dependent care (only for the dependent care FSA); or
- Loss of a dependent (death).
To change your benefits, notify the People Team within 30 days of the qualifying life event, and provide proof of the change, such as a marriage certificate, birth certificate, or student verification information.
For medical, dental, vision, HSA or FSA elections, contact the People Team to request a change. They will then guide you through next steps of the enrollment process.

This is a summary of the annual report of the IncredibleBank and Related Employers, EIN 39-1440245, Plan No. 501, for period 01/01/2023 through 12/31/2023. The annual report has been filed with the Employee Benefits Security Administration,
U.S. Department of Labor, as required under the Employee Retirement Income
Security Act of 1974 (ERISA).
IncredibleBank has committed itself to pay certain self-insured Medical and Dental claims incurred under the terms of the plan.
Insurance Information
The plan has contracts with Wyssta Insurance Company Inc and Symetra Life Insurance Company to pay Vision, Life Insurance, Long-term Disability, Accidental Death and Dismemberment, and Employee Assistance Program claims incurred under the terms of the plan. The total premiums paid for the plan year ending 12/31/2023 were $138,448.
